Do you want to contribute to ensuring that patients discharged from hospitals and short-term care facilities have a safe transition back home? We are now looking for occupational therapists who want to help shape a new way of working. Our guiding principle is that a safe return home should be built on clear roles, realistic conditions, and shared responsibility – with a focus on those who need it most. Help us make a difference!

About the role

When a person leaves the hospital or short-term care, it is often one of life's most vulnerable transitions. At that moment, support needs to be as clear, secure, and coordinated as possible. Therefore, we want to develop our working methods to create a cohesive chain where we meet the person early, in a coordinated manner, with a shared focus: the right support at the right time, in their own home. We aim to create security for the individual and strengthen the quality of the entire care chain.

You will be part of the "Safe Home Return" team and collaborate with nurses and social workers. The goal is to assess and plan for new or expanded interventions from healthcare and social care. You will contact the individual and/or their legal representative via phone or meetings at the ward. Together with the rest of the Safe Home Return team, group leader, and unit manager, you will develop working methods and approaches to be used in care planning to contribute to knowledge, relationships, participation, and continuity for the individual.

Examples of tasks:

  • Develop working methods and approaches for the discharge process together with the team
  • Investigate the need for occupational therapy according to the occupational therapy process
  • Plan, implement, and follow up on rehabilitation measures
  • Maintain contact with and provide support to relatives and close ones
  • Collaborate with other professions to create individual care plans
  • Document in various quality registers and operational systems

About the workplace

You are part of the Authority Operations and are based at Olof Palmes torg. The group will consist of a nurse and an occupational therapist, with close collaboration with social workers. You will also be present at the hospital and within home care services.

Qualifications

You have the ability to establish good contact with other people, work well in teams, and strive towards common goals. You work from a salutogenic approach with a focus on participation and quality of life to see the individual's resources and create security in connection with discharge. You are structured, prioritize well, and have a forward-thinking mindset, wanting to contribute to the development of our operations. Furthermore, you are flexible and have the ability and willingness to find new solutions. You master the Swedish language to a level where you can communicate with patients and other staff, as well as document what the operation requires. As the work involves digital documentation in operational systems, good computer skills are required.

Requirements:

  • Registered occupational therapist
  • Valid driver's license (Category B)

Merits:

  • Experience working as an occupational therapist
  • Knowledge and/or experience of the discharge process
  • Knowledge of regulations such as the Social Services Act, the Health and Medical Services Act, and the Act on Support and Service for Certain Disabled Persons
  • Geriatric training
  • Knowledge and experience with the documentation system LifeCare

Scope and start date

Employment type: Permanent position

Full-time: 100%

Start date: As soon as possible, according to agreement

Number of positions: 1-2

Working hours: Monday-Friday, daytime
