Region Örebro County is developing primary care approaches for patients with extensive and complex care needs. The growing proportion of elderly residents, especially those over 80 years old, creates a need for enhanced continuity, comprehensive assessments, and coordinated interventions.
Varberga Health Centre is now looking for a nurse or district nurse with a special interest in geriatrics and care coordination. We offer:

Job Responsibilities
The position involves a combined role consisting of clinical patient work and responsibility as a care coordinator. The clinical work includes:
- Emergency and planned clinic operations
- Telephone advice and medical assessment
- Digital patient contacts
- Home visits
The work is carried out in close collaboration with other professions at the health centre.
As a care coordinator, you are responsible for:
- Planning, coordinating, and following up on care interventions based on patient needs
- Ensuring continuity and patient safety in the care chain
- Leading coordinated individual planning (SIP)
- Collaborating with internal and external actors according to the current collaboration model (ViSam)
- Contributing to the development of working methods for frail patients
The work is characterized by a structured, independent, and collaborative approach.
Qualifications
Requirements:
- Licensed nurse or district nurse
- Good ability to conduct medical assessments
- Ability to communicate clearly and professionally, including via telephone
- Experience working in primary care or with elderly patients
Merits:
- Further education in geriatrics and/or cognitive impairment
- Experience in care coordination or coordinated care interventions
You are confident in your professional role and have the ability to work both independently and in a team. You have a structured working style and good collaboration skills. Furthermore, you are attentive to the needs of patients and relatives and have a professional manner of dealing with people.
