Do you want to work in a role where the mission is to strengthen the collaboration between Askersund Health Centre and Askersund Municipality? We are now looking for a coordinator who will contribute to smoother transitions, better follow-up, and more cohesive care for patients who need interventions from multiple services.

The position is located at Askersund Health Centre, but the role is shared with the municipality, meaning you will work in both organizations. You will help establish a working method that makes it easier for the health centre and the municipality to meet around the patient, monitor needs in a timely manner, and reduce the risk of patients falling between different services.

Project-based employment, full-time, located at Askersund Health Centre within Region Örebro County. The role is shared between Askersund Health Centre and Askersund Municipality, meaning part of the work is performed within the municipality's health and medical care. The position is jointly funded by the organizations. The planned start date is October 1, 2026, and the position runs until March 31, 2027. The employment is valid for the project period and may lead to a permanent position if necessary decisions are made.

Job Description

As a care coordinator, you work to strengthen the contact between the health centre and the municipality's health and medical care. The role involves creating better flows, clearer communication, and safer transitions for patients who often interact with several parts of the healthcare and social care systems.

The role includes coordinating contacts between the health centre and the municipality, providing support during discharges from inpatient care, and ensuring patients are picked up at the right time. You may also conduct home visits and follow-ups based on assignments, needs, and your primary profession.

The work involves close collaboration with care coordinators, nurses, doctors, rehabilitation professionals, discharge teams, and other relevant functions. The role includes, among other things, working based on HSL prescriptions and delegations from various professions within the rehabilitation team.

Since the role is new, you will also be involved in shaping working methods, routines, and collaboration pathways between the organizations. This requires the ability to create structure, see the whole picture around the patient, and contribute to making collaboration work in practice.

Qualifications
  • Education as a nurse, occupational therapist, physiotherapist, or assistant nurse
  • Experience (broad) in health and medical care
  • Good understanding of collaboration between different healthcare providers

To succeed in the role, you need to be confident, receptive, and have an easy time collaborating with others. You must be able to work independently, create structure in situations where multiple actors are involved, and communicate clearly with patients, relatives, and colleagues. You have the ability to see the whole picture around the patient and contribute to trusting relationships between the health centre, the municipality, and other partners. We will place great emphasis on your personal qualities.

Merits:

  • Experience working in collaboration between region and municipality
  • Experience with discharge processes from inpatient care
  • Knowledge of the VISAM process or other methods for coordination and safe care transitions
  • Experience from primary care, municipal health and medical care, or discharge teams
  • Experience in coordinating roles
