Care Manager Nurses Wanted in Askersund Municipality

The Social Services Department in Askersund Municipality is responsible for elderly care, LSS (Support and Service Act for Persons with Certain Functional Impairments) and care operations, social psychiatry, operations for unaccompanied refugee children, as well as individual and family care.

The Health and Medical Care Unit also falls under the Social Services Department and is undergoing exciting development where we are investing in new roles to strengthen operational quality work for our patients. Therefore, we are also seeking Care Manager Nurses for both home healthcare and specialized housing!

2 positions.

Job Responsibilities

Care/Nursing Science

  • Assesses the needs and resources of the patient and relatives from a salutogenic and holistic perspective.
  • Notices and acts upon changes based on changes in health status.
  • Supports the patient's participation in care and self-care ability.
  • Reflects on and takes measures to treat, alleviate disease and symptoms in relation to the patient's life world.
  • Works with quality and operational development.
  • Provides specific care and symptom relief at the end of life.
  • Critically evaluates different treatment methods.
  • Applies a team-based approach to meet the needs of patients and relatives, offering supportive and advisory conversations.
  • Works from a holistic view with the patient/relatives/team.
  • Leverages the knowledge of the multidisciplinary team.
  • Takes advantage of the patient's own resources for the best experience of well-being.
  • Is responsible for medical-technical aids used in home healthcare.

Interaction, Information, and Education

  • Uses pedagogical techniques to strengthen the patient's decision-making.
  • Initiates and conducts difficult conversations with patients and their relatives.
  • Supervises, teaches, and assesses students from a professional perspective.
  • Supervises care work and educates care staff.
  • Respects the autonomy, integrity, and dignity of the patient/relatives.
  • Participates in ethical discussions and decisions.
  • Implements and arranges training regarding delegation according to current regulations and guidelines.
  • Conducts own development work and stays updated with new research findings.

Safety & Quality

  • Works independently based on relevant national guidelines, local care programs, and national quality registers.
  • Identifies factors that pose a threat to safe care and works systematically to prevent care injuries.
  • Handles patient information in a way that patient safety and confidentiality are always guaranteed, despite continuity breaks due to various legal provisions and operations.
  • Works according to current laws, regulations, as well as guidelines and standards regarding home healthcare.
  • Creates a purposeful work and care environment.
  • Identifies needs and initiates contact with physiotherapists and occupational therapists to ensure the patient receives the right aids and housing adaptations.
  • Initiates, leads, and participates in quality development.

Qualifications

We are looking for you who:

  • Are a licensed nurse or specialist nurse.
  • Have experience in municipal health and medical care.
  • Have a good understanding of laws and guidelines within the Health and Medical Services Act (HSL).
  • Preferably have experience in quality development and improvement work.

Personal Qualities

  • Development-driven and solution-focused.
  • Flexible, structured, and team-oriented.

The position requires a valid driver's license (Category B).

Salary: Monthly salary.
Employment type: Permanent position.
