Centrum Health Center is a developing unit with approximately 13,000 registered patients, making us one of the largest health centers in Skåne. Here, a multidisciplinary team works together, including specialist doctors, resident doctors, nurses, assistant nurses, counselors, psychiatric nurses, occupational therapists, physiotherapists, and dietitians.

RESPONSIBILITIES

As a diabetes nurse with us, you will meet various types of diabetes patients and work with, for example, adult patients with diabetes, newly diagnosed diabetics, and patients with prediabetes. You will conduct planned follow-ups of our diabetes patients and make registrations in the National Diabetes Register (NDR). You will be engaged in the development of our diabetes clinic with high medical quality and professionalism. You are expected to stay updated on diabetes knowledge, maintain close collaboration with doctors, dietitians, and others at the health center, to offer patients evidence-based diabetes care.

To support your professional development, we offer regular and relevant training.

In addition to your own clinic work with diabetes patients, the role also includes other duties at the health center, such as telephone advice and triage.

QUALIFICATIONS

We welcome you who are a licensed nurse or district nurse, with the authority to practice within the Swedish healthcare system. Additionally, you must have further education as a diabetes nurse. Language skills in Swedish corresponding to at least level C1 according to the Council of Europe's scale are a requirement. Furthermore, it is an asset if you have work experience from primary care and/or experience working in the PMO medical record system.
