Social Worker with a Focus on Outreach Work
Are you interested in a position as a social worker in an organization with multiple areas of responsibility? We are expanding our operations with a social worker focused on outreach work to further meet the needs of our community residents and as part of the organization's transition to a new social services law. The position is located within the Employment and Establishment Authority in the Individual and Family Care department in Vellinge Municipality. The department includes housing coordinators and social workers who handle financial assistance, social psychiatry interventions, budget and debt counseling, housing interventions, family law, domestic violence, and substance abuse and dependency, with a total of 11 employees. Our staff have high expertise in their respective areas, and the workgroup collaborates, supports each other, and covers for one another as needed. The workgroup you will belong to is led by a unit manager.
JOB DESCRIPTION
As a social worker focused on outreach work, you will support and guide individuals experiencing homelessness, substance abuse, and/or domestic violence in developing their ability to achieve independence and participation in their life situation. Your task will be to provide additional support during periods when other support measures are insufficient, for example, in managing daily life (in housing and society) and maintaining sobriety/drug-free status.
The assignment involves conducting outreach work, which means that much of the work is carried out outside of an office environment. Together with your colleagues in the authority's operations, you are responsible for providing high-quality services that are planned and executed for the individual.
The work is varied and involves providing both practical and social support in daily life based on each individual's needs and implementation plan. In your outreach role, you will collaborate with the general psychiatry FACT team (Flexible Assertive Community Treatment) in Trelleborg. FACT involves a multi-professional team that offers individuals with severe mental illness integrated and comprehensive services in long-term, outreach, and flexible forms. You are also expected to establish closer collaboration with other care providers, such as health centers.
We work from a salutogenic perspective with low-arousal responses. The job requires a high degree of personal responsibility and initiative. The work may involve solitary work with the individual.
The employment is a three-year project position. Since the position represents a reinforcement in the organization, the tasks may vary based on the needs that arise.
Examples of Common Tasks:
- Upon return home, provide support in following through with their planning after completed treatment. Support can be provided both at the treatment facility and in the home based on individual needs.
- Provide support and motivate for activity, employment, and participation, and be present in daily life when needed.
- Basic budget and debt counseling.
- Facilitate and support colleagues' outreach work, e.g., during LVM reports/welfare reports.
- Guidance in daily tasks such as cleaning, laundry, shopping, cooking, and paying bills.
- Participate in walks, training, outings, and errands.
- Guidance on personal care such as hygiene and routines related to this.
- Conduct documentation and follow up on interventions.
- Authority operations within one or more areas of activity.
QUALIFICATIONS
Requirements:
- Post-secondary education of at least two years in social work, behavioral science, or equivalent. This may include a social worker, social pedagogue, or experience deemed equivalent by the employer.
- You have good computer skills and can express yourself well in Swedish, both verbally and in writing.
- Driver's license (B).
Desirable:
- Work experience from similar assignments within the target group.
- Experience working with individuals with comorbidity issues and/or domestic violence.
- Knowledge of low-arousal responses and clarifying pedagogy, as well as a solution-focused approach.
- Experience working with authority operations is desirable but not required.
