Adult habilitation in Halmstad is looking for a social worker for our adult team

Job Responsibilities

We work in a cross-professional team with a person-centered approach, which means that we put the individual with their unique needs, abilities, and life stories at the center of care. The work is conducted at the reception and, if necessary, also outside the reception. The team consists of a social worker, occupational therapist, speech therapist, psychologist, physiotherapist, nurse, and special educator.

As a social worker, you will work with psychosocial treatment, provide information about social support and social legislation. You will also convey knowledge about the disability to the patient and relatives. The assignment includes collaborating with and facilitating contact with other services, as well as being a knowledge resource. A well-planned introduction is, of course, essential for us, and you will have the opportunity for continuous support from colleagues at your workplace and in the region. At Habilitation, you will have regular supervision for both the team and professional group.

About the Workplace

The habilitation in Halmstad is one of four receptions in Halland that provides habilitative services to children, youth, and adults with various forms of disabilities. The reception in Halmstad is divided into three teams, two of which work with children up to 18 years old and one with adults from 18 years old. The catchment area is Halmstad/Laholm/Hylte with placement in Halmstad. The reception has a wealth of experience and a desire for continuous development to meet patients' needs in the best way. A leasing car is available. We care about a good working environment.

The adult habilitation is HBTQI-certified.

Qualifications

We are looking for someone with a degree in social work. Experience in habilitation, teamwork, and working with people with disabilities is advantageous, as well as planning and conducting group activities and supportive conversations.

The adult habilitation is responsible for Halmstad, Hylte, and Laholm, so a B driver's license is a requirement. A leasing car is available. Primarily, you will have your workplace in Halmstad and will work from there.

We are looking for someone with good collaboration skills, the ability to plan and structure your work, take initiative, and be flexible. You have an empathetic approach with the ability to maintain professional boundaries. You enjoy being involved in common issues at the workplace and contributing to continuous improvements. Computer skills are required. We place great importance on personal suitability.
