In primary care rehabilitation, all rehabilitation resources in the public primary care are gathered. Here, we work as physiotherapists, psychosocial resources, occupational therapists, rehabilitation assistants, and rehabilitation coordinators. Together, we are responsible for rehabilitation and treatment of physical and mental health issues at the county's public health centers.

We are now looking for an engaged rehabilitation coordinator who wants to work across five health centers and, together with a colleague, develop our coordination assignment. Here, you will have a central role in coordinating efforts around patients in the sick leave and rehabilitation process. The placement is intended at Dalbo Primary Care Rehabilitation, a workplace with good cooperation and quick contact paths. Here, we work to ensure that our patients receive quality care and feel secure and trusted. RESPONSIBILITIES

As a rehabilitation coordinator, you are responsible for creating structure and continuity in patients' return to work. You coordinate contacts between healthcare, the Social Insurance Agency, the Employment Service, and other actors. The assignment also includes individual coaching of patients, often involving motivational interviewing to strengthen their own ability in the rehabilitation process. Together with your colleague, you are responsible for the health centers Rottne, Braås, Åseda, Lenhovda, and Dalbo.

QUALIFICATIONS

We are looking for someone who has:

• Experience in insurance medicine, sick leave processes, and the rehabilitation chain
• The ability and habit of coordinating efforts between different societal institutions
• Relevant higher education, e.g., as a social worker or equivalent.
• A structured working method and good communicative ability
