The Social Administration is organized into four departments and a quality and development department. The Social Administration is organized into authority functions for the elderly and people with disabilities, individual and family care, as well as support, advice, and other interventions for children, youth, and adults. The Social Administration has its own management for elderly and disability care.

In the elderly department, 28 people work. The group you will belong to consists of area-responsible social workers, discharge planners, social workers responsible for follow-up in special housing, a person responsible for outreach work, housing coordinators, caseworkers for transport services, national transport services, senior housing/safety housing, and group leaders. The department also includes caseworkers for housing adaptation grants, fee caseworkers, and administrative assistants, as well as the preventive unit with relative consultants, dementia coordinators, civil society coordinators, safety housing coordinators, and senior meetings, welfare technology project leaders, vision and hearing instructors, and group leaders. The department manager leads the department.

JOB RESPONSIBILITIES

As a social worker/discharge planner, you will plan and coordinate interventions related to discharge from the hospital for the elderly target group or other ages in need of interventions according to the Social Services Act. The most common interventions are home care, nursing and care homes, short-term accommodation, and day activities. IBIC - the individual's needs in focus is applied. Your task will be to investigate, assess, and make decisions about interventions according to the Social Services Act and follow up on short-term accommodation. You will also have contact with clients and their relatives and have close cooperation with other social workers and group leaders. In addition, you will also collaborate with our providers, primary care, and inpatient care. A big plus if you are interested in taking on areas of responsibility as we develop our work.

Our goals focus on Danderyd Municipality's strategic areas of high quality, great freedom of choice, high efficiency, high attractiveness, and strong development. Development areas that you and the department will be involved in are the new Social Services Act, the municipality's digital strategy and welfare technology, knowledge-based, preventive approaches, an outreach and even more accessible social service. Currently, we are working on Individual-Based Systematic Follow-Up (ISU) and improving our operational system.

QUALIFICATIONS

We are looking for you who:

  • are a social worker or have another academic education deemed equivalent.
  • have good knowledge of current legislation.
  • have good knowledge of Swedish, both spoken and written.

It is an advantage if you have several years of experience working as a caseworker in elderly care recently.

You show great commitment and interest in the target groups. You are confident in your professional role, attentive, and solution-focused. You are flexible and can easily adapt, can handle work peaks, and accept guidance to help prioritize. You have good habits of working structured and independently. You express yourself well in speech and writing and have very good collaboration skills. You enjoy developing the area. Great emphasis will also be placed on personal suitability.
