Do you want to be part of our exciting journey towards establishing a Mobile Primary Care Team in the northern part of the county? We are starting a pilot project in the spring of 2026 aimed at providing residents in the northern part of the county with care at the right level, increasing safety, and improving access to care through a collaboration team between the municipalities in the north and Region Örebro County.
The Primary Care Team is a cohesive form of care where primary care, municipalities, and hospitals combine their efforts through collaboration to offer safe, clear, and accessible care for patients who cannot reach the health center and who require medical care that is better provided at home than in the hospital. The team can assess, plan, prescribe, and perform interventions. Care is specialized based on each individual patient's needs, and the goal is to provide qualified care both medically and psychosocially.
We are looking for an enthusiastic and development-oriented nurse for an exciting assignment in the Primary Care Team. You will work independently but will also be an important part of a team. Nurse, Primary Care Team Northern CountyJob ResponsibilitiesThe responsibilities in the Primary Care Team are varied and include medical assessments over the phone and in the patient's home, planned and more urgent treatments, medication management, nursing, and patient work. In the Primary Care Team, you work in a team, and the mobile assignments are carried out together with a nurse or doctor. The assignments are conducted in collaboration between the Primary Care Team, hospital services, municipalities, and health centers in the northern part of the county.
Your position involves working in a team where you make home visits to patients in different municipalities. You lead and actively participate in planning around the patient's care together with other professionals and collaborate with other care providers. In cases where the patient may need hospital care, we strive for direct admissions to the ward. Participation in the development and improvement work of the operation is, of course, also part of your responsibilities. QualificationsLicensed nurse. Experience in similar tasks is advantageous. A valid B driver's license is also a requirement as care is largely provided in the patient's home. We place great importance on your personal suitability and ability to be flexible. You should have good collaboration skills and social competence. We see that you are open to changes and have the ability to work under varying workloads.

Employment Information:
We see it as an advantage if you retain your existing permanent employment and share your working hours between these operations for the best possible collaboration; therefore, there is an opportunity to work in the team as part of your existing employment.
During the project, you will work Mon-Fri, daytime. Our hope is that the pilot project will be made permanent, and steps to achieve this will be taken continuously.
