The rehabilitation hotel is located in the premises of the Old Karolinska Hospital (GKS) in Solna and provides ASiH (advanced home care) and SPSV (specialized inpatient care) on behalf of Region Stockholm.

We work in a multiprofessional team consisting of nurses, doctors, nursing assistants, physiotherapists, occupational therapists, dietitians, and social workers. Upon admission, we have a comprehensive responsibility for the patient's situation, along with specialist competence regarding healthcare and rehabilitation. Patient categories within our operations include: heart disease, COPD, asthma, cancer, infections, hematology, and nutritional issues.

Our patients live in central Stockholm, within the toll ring, Hammarby Sjöstad, Lidingö, and Solna.

We are looking for a social worker who wants to be involved in the patient's care with a holistic view and understanding of the situation the patient and their relatives are in. You enjoy collaborating in a team to achieve the best possible care for the patient, where the patient's best interest is always the focus. You are flexible and used to solving problems, both independently and in dialogue with other professionals.

Qualifications: good knowledge of the Swedish language, both spoken and written. Desirable: Experience from ASIH, palliative care, home care, and geriatrics.

Job Responsibilities

The work involves both visiting patients in their homes and working in our SPSV department. Job responsibilities include psychosocial support, support conversations for patients and relatives, as well as support conversations for mild depression and anxiety. It also includes standard practical social support measures with contact with social service caseworkers, home care, etc., as well as collaboration with other healthcare providers.

The job also includes being a support for the collegial team in the form of individual conversations in special situations and group discussions, such as ethical conversations.
