Home Care Mellersta is based in Ekbacka in central Borgholm and serves care recipients from Köpingsvik to Föra in the north. We are divided into smaller workgroups within the team, and we work based on a person-centered and rehabilitative approach, focusing on continuity and quality in our work to provide good care and support.

JOB RESPONSIBILITIES

Working in care means that you truly make a difference in other people's daily lives. As a care assistant in home care, you will have varied tasks and work according to a rehabilitative approach where the care recipient's needs and life situation influence how tasks are performed. Based on individual assistance decisions, you will support individuals who have been granted home care to manage their daily living. Care recipients may need assistance with personal care, social activities, service tasks, and medication management.

Your responsibilities as a care assistant also include being a primary contact for care, establishing and following up on implementation plans, and documenting in the Lifecare journal system. Collaboration in interdisciplinary teams is a significant part of your work, as well as independent work where you take personal responsibility.

We use digital tools where we register visits, sign medications, and document. You will work in accordance with the Social Services Act (SoL) but also perform health and medical care tasks (HSL assignments) after delegation from licensed personnel.

Working hours are distributed across days, evenings, and weekends, and you have the opportunity to propose your own schedule within given frameworks.

QUALIFICATIONS

We are looking for someone who:

• is a trained care assistant.
• has a B driver's license
• has excellent knowledge of Swedish, both spoken and written.
• has a genuine interest in working with people and can meet each individual based on their unique needs and wishes.
• is positive, flexible, has a respectful and professional demeanor, and good collaboration skills.
• works according to a rehabilitative approach.
• takes responsibility for your tasks and shows initiative and engagement.
• has necessary computer skills, preferably in Lifecare

Experience with Senior Alert, BPSD and working in Lifecare is also meritorious.
