Österåker Municipality is looking for a dementia nurse who, with competence, commitment, and heart, wants to contribute to realizing the national dementia strategy 2025–2028.

Your new job

This is a role for you who want to combine strategic thinking with practical action, motivated by contributing to quality work that makes a difference for both the organization and individuals with cognitive diseases and their relatives.

The dementia nurse collaborates with staff within care and support in teams and is to work according to the national guidelines for care and support for dementia and follow the national target levels. You work closely with the department head, unit managers, and the medical responsible nurse. As a dementia nurse, you lead the work with the national dementia strategy and are responsible for developing and implementing the municipality's collaboration routines for dementia.

Training staff is an important part of the work, as is supervision, and sometimes this work is done directly in patient meetings. Through good collaboration with general practitioners, you gain knowledge about patients early in the disease and follow them, together with care managers, according to the standardized intervention process. You lead cross-professional dementia teams within the authority that ensure quality assurance and development of the operations. Day activities are an important part of the work, where regular evaluations and development work take place.

Support for relatives is provided in collaboration with a relative consultant and consists of information, study circles, cafes, and support conversations. The work is very varied, and you are responsible for planning and prioritizing together with the department's management group.

As a dementia nurse, you will be a key person in the work to:

• Ensure person-centered care and support.
• Strengthen early detection and diagnosis in collaboration with primary care and relatives.
• Develop knowledge and competence among staff and relatives.
• Develop the municipality's governing documents and routines in the dementia area.
• Contribute to coordination and continuity throughout the care chain.

Your profile

We are looking for you who are a specialist nurse with a focus on care for dementia. The assignment includes supervising and training staff. It is meritorious if you are an authorized nursing supervisor or have some form of supervisory training as well as pedagogical education.

You can express yourself well in both spoken and written Swedish.

As a person, you are cooperative, solution-oriented, and unpretentious. You have an easy time understanding others' situations, listening, understanding, and showing consideration.

A driving license (B) is a requirement.
