Nurse for En Väg in

In Hässleholm municipality, we have revamped our approach to En Väg in.

Working as a nurse in En Väg In means you are the first contact for patients seeking home healthcare. You receive incoming cases via referral, assess the need for home healthcare through home visits. The role also includes monitoring patients in the hospital who are not already known to the municipality through Mina Planer, and is central to creating a safe, efficient, and coordinated care.

Part of the position will involve clinical work where you will be a resource in one area.

Working hours are daytime, Monday–Friday (40 hours/week). Two public holidays per year are distributed fairly based on previously worked weekends.

Job Responsibilities

  • Assessment of eligibility for home healthcare through direct patient visits.
  • Coordination and planning of interventions with the municipality, hospitals, and primary care via Mina Planer for patients who are not previously known in the municipality
  • Information to patients and relatives, documentation, and care planning.
  • You will be a resource in one area for the clinical part.
  • The work is varied, both administrative and direct patient contact.

Who are you?

  • Are a licensed nurse
  • Have a warm demeanor and a genuine interest in patient-centered work.
  • Thrive in a coordinating role and in collaboration with multiple stakeholders.
  • Are confident in clinical assessments and can work independently.
  • Have good computer skills and command Swedish in speech and writing.
  • We prefer that you have good experience documenting in the journal systems Procapita and Lifecare, as well as experience working in the systems Mina Planer, NPÖ, and Pascal.
  • Have a B driver's license

Meritorious: Experience in home healthcare, geriatrics, inpatient care, care planning.
