JOB RESPONSIBILITIES

The Department of Health, Care, and Prevention consists of home care and home health care. To find the best individual solutions, we work in interdisciplinary teams. The development work focuses on digital solutions that simplify everyday life for employees and help ensure good quality for citizens.

As a dementia nurse with us, you will be part of our management team together with the unit managers and have close contact with MAS/MAR.

In 2026, we will be one of the first municipalities to be Star Rated in all operations: authority, special housing, home care, LSS. A significant and important investment.

Today, there is also a very good collaboration with the health centers we work with, and you, who take on the role, will be a key person in the care of those with cognitive impairment.

The role of the specialist nurse/Silvia nurse means functioning as a consultant within the municipality and having a holistic perspective where the work area includes, among other things: Collaboration, Education, Supervision, Knowledge bearer, Community informant, Inventory and evaluation, and Individual support.

The role of the specialist nurse/Silvia nurse includes:

• Being part of the Cognitive Specialist Team in the municipality
• Being a guide for the person with cognitive illness and their relatives throughout the disease process.
• Providing support and advice to the operations in Burlöv municipality for counseling, supervision, and education on issues related to cognitive illness.
• Working with family support and in close collaboration with the municipality's family coordinator.
• Being a face in the community that citizens can turn to with questions about cognitive illness.
• Collaborating internally and externally for a better and smoother care chain for people with cognitive illness.
• Being a knowledge bearer in the organization, staying updated on new research, etc., to maintain quality in care and support.
• Working on development issues based on the National Guidelines from the National Board of Health and Welfare for the care of people with cognitive illness/dementia and other governing documents.

QUALIFICATIONS

Nurse with specialist education in cognitive illness or Silvia nurse.
