JOB RESPONSIBILITIES

If you want to help develop Safe Homecoming with a new initiative, IHR Intensive Home Rehabilitation, then you should apply to us. We are in the starting blocks to begin working with the IHR initiative where you, as a physiotherapist, will be an important part of the team. You, together with your occupational therapist colleague, will conduct baseline measurements and together with the customer/patient and team set goals in the form of SMART goals for the customer’s/patient’s training. The training takes place over a longer period, a maximum of 3 months, to conduct new measurements before completion when the goals have been achieved. The goal is for customers/patients to become as independent as possible without help from home care when we finish. At the same time, we continue to work with Safe Homecoming customers/patients who need to be assessed for a shorter time regarding what help they need from home care. All our customers/patients come from the region, short-term care, or directly from home.

You will become part of a professional team consisting of occupational therapists, nurses, specialist nursing assistants, and coordinators. Everyone works in close teamwork with meetings every weekday afternoon. We sit in the same premises and have short communication paths. A teamwork where everyone is an important part of making it work for those we are here for, our customers/patients.

QUALIFICATIONS

Licensed Physiotherapist

We are looking for a licensed Physiotherapist who has preferably worked for a few years. You should be flexible with good collaboration skills, as you will be working with many different actors simultaneously.

We would like you to have experience in municipal, regional, and/or primary care assignments.

Your work area is the entire Enköping municipality.

You easily adapt to changing circumstances. You see opportunities in changes and can quickly change your perspective and approach based on what the situation requires.

You collaborate well with others and relate to them in a responsive and smooth manner. You listen, communicate, and resolve collaboration difficulties or conflicts constructively.
