We are looking for a new employee
We are looking for a new employee for an organization characterized by commitment and professionalism. The work is varied and stimulating, with the opportunity to meet many people and contribute to a safe and meaningful everyday life for the municipality's residents.
The organization primarily targets elderly individuals, but also younger individuals with various disabilities. The Social Services Act is the main legislation we work under, which provides good conditions to identify needs together with the individual and design interventions that enhance the individual's quality of life.

Job Responsibilities
- Be responsible for planning and coordinating discharges from hospitals and short-term care in collaboration with the region, internal, and external actors
- Ensure that decisions are made promptly and with good quality
- Work flexibly and adjust planning based on changing needs and conditions
- Represent the organization professionally and ensure that the authority's exercise of power is carried out correctly and in accordance with applicable regulations
- Process cases, make, and follow up on decisions according to the Social Services Act
The position requires the ability to handle multiple individual cases with varying degrees of complexity simultaneously.
The assistance unit continuously works to develop the organization, and the person we are looking for should be involved and contribute to development and feel engaged in the change process.
Qualifications
The following is required for the role:
- Degree in Social Work or other higher education combined with experience deemed equivalent by the employer
- Good digital skills and experience in documenting and working in Treserva
- Experience in needs assessment, focusing on the elderly
- Experience working with SIP, care planning, and discharge planning
- Experience working in IBIC, Individual's Needs in Focus
- Driving license B

In the role of case officer, you will meet new individuals daily who find themselves in more or less vulnerable situations. Therefore, it is important that you show interest and understanding for the clients you meet and work towards good communication and collaboration with colleagues, relatives, and other professionals working around the client. You have an independent working style, are structured, and have a good ability to organize and prioritize your work effectively. Furthermore, you are flexible and decisive, adapting to changing circumstances and driving processes in a goal-oriented manner. Since the work involves contacts with many partners, you should have very good collaboration skills.
