As a rehabilitation coordinator with us, you will play a key role in supporting individuals back to work at three of our clinics in Täby.

You coordinate, guide, and motivate – ensuring that healthcare, employers, and the social insurance agency work towards the same goals.

Your Responsibilities

• Coordinate rehabilitation efforts for patients on sick leave at three of the clinics (HLM in Täby C, Vallatorpsdoktorn, and Täby Kyrkby HLM). Provide support to both healthcare staff and patients in the rehabilitation process.
• Follow up on rehabilitation plans. Maintain contact with the Social Insurance Agency, employers, the Employment Agency, municipal authorities, etc.
• Contribute to the development of rehabilitation processes within the organizations.

We are looking for someone who

• Has a relevant university degree in areas such as healthcare, behavioral science, or social work.
• Has experience in rehabilitation work and preferably in coordination.
• Is structured, communicative, and has good collaboration skills.
• Has knowledge of the social insurance system and the roles of various stakeholders.
• Can see both the organizational and patient perspectives holistically.
• Has good communication skills and the ability to build positive relationships with both patients and colleagues.
• Is attentive to our patients' total needs and approaches everyone with a professional and humble attitude.
• Is stable, adaptable to different situations, and works well both in teams and independently.
• Enjoys taking responsibility and showing the commitment required in a small unit.

Job Duties

The Rehabilitation Coordinator supports sick-listed patients in the rehabilitation process and facilitates their return to work or entry into the workforce. The coordinator's support and coordination also serve as a resource for doctors and other health professionals in rehabilitation and sick leave matters. We work in teams at our three clinics.

The Rehabilitation Coordinator has three areas of responsibility:

• Support for the patient
• Internal coordination of rehabilitation and sick leave
• Collaboration with external stakeholders, such as the Social Insurance Agency and employers.

Qualifications

Licensed nurse, occupational therapist, or social worker with education from – Karolinska Institute's course "Coordinating Efforts - Coordinating Sick Leave & Rehabilitation Processes (7.5 credits)" or have equivalent knowledge of insurance medicine.
