We are a region-driven player in palliative home care and SPSV in Stockholm County.

On the northern side of Stockholm, we have our ASIH operations in Järfälla/Upplands-Bro, Solna Strand, and Danderyd, as well as our own specialized care department (SPSV) at Jakobsberg Hospital. We offer safe, person-centered, and qualified care – both at home and in the department, where we work with end-of-life care. We work in multiprofessional teams with the goal of creating conditions for good care based on the patient's needs, together with the patient and their relatives. We value initiative, care, and curiosity.

We are now looking for a social worker to join our unit in Danderyd, who wants to be part of our interprofessional team that enables severely ill patients to stay at home despite significant care needs.

Responsibilities

As a social worker with us, you will focus on the well-being of patients and their relatives. The work is both varied and independent while you are an integral part of the teams working around the patients. You will provide psychosocial support, counseling, and advice, striving to strengthen the resources of patients and their relatives. Your tasks will include conducting assessments, participating in care planning, and collaborating with various stakeholders around the patient.

Qualifications

To succeed in your role as a social worker with us, you have a degree in social work or another equivalent higher education that the National Board of Health and Welfare deems suitable for social work in health care. In our organization, we place high value on qualities such as security, responsibility, humility, communicative willingness, curiosity, and respect. You have a holistic view of people and see psychosocial care as an important part of the work. You find it easy to collaborate with colleagues, patients, and relatives. A driver's license (B) is a requirement. Experience in ASIH or other healthcare experience is an advantage.

Additional Information

Since patient/relative contact primarily occurs in the patient's home, you cannot be overly sensitive. Both pets, smoking, strong odors, and allergens may be present.
