About the Position

Do you want a meaningful job where you make a difference for citizens in an important transition between hospital and home? The Case Management Unit, Care Sector in Kävlinge Municipality is now looking for a substitute SVU Coordinator for a colleague who will be on parental leave.

As an SVU Coordinator, you collaborate with inpatient departments and primary care before discharges from the hospital. You work in the region-wide IT support system Mina Planer to ensure safe homecomings for citizens who may need care support and daily rehabilitation.

The work is varied and includes:

• Collaboration with licensed colleagues, home care areas, the homecoming team, and the short-term unit.
• Administration and communication via phone and email with citizens, relatives, and partners.
• Authority exercise in the form of simplified investigations according to the Social Services Act in the LifeCare Case Manager system.
• Participating in MAS/MAR meetings and representing the unit in various collaboration forums.

You will be part of an engaged team together with two SVU case managers and participate in multiprofessional meetings every week.

Are You the One We Are Looking For?

We are looking for someone who is:

• A licensed nurse or licensed occupational therapist.
• Has at least 5 years of experience in municipal operations (home healthcare and/or care).
• Structured, clear, and confident in your profession.
• Used to working administratively and enjoys phone contact.
• Capable of prioritizing, coordinating, and building good relationships.

You are committed to collaboration and can handle periods of high tempo without losing quality.

About the Employment

The position is a temporary position for a parental leave with a start date of 2026-04-01 or by agreement. In this recruitment, we will apply ongoing selection.
