Planner for Home Care for Younger People in Lund Municipality

About the Role and Your Responsibilities

You will be employed in a regular nursing assistant position with working hours during the day (06:45–15:15) and working every other weekend. The role includes both administrative tasks and work in daily care where you will be part of the team.

You are responsible for planning and scheduling the approved services for our clients. You will have regular contact with clients, relatives, managers, health care staff, colleagues, and caseworkers. In your work, you will use Lifecare and My Plans.

You will work closely with our coordinator who is on-site Monday to Friday. The planner role is staffed by two people, and you will take turns working in care every other weekend.

This position is a permanent employment as a nursing assistant. The planner assignment runs out every six months, with good opportunities for extension.

We offer full-time employment with a weekly working time of 36 hours and 20 minutes. In your daily work, you are an important part of the clients' everyday lives and contribute to enabling them to live as meaningful, independent, and active lives as possible.

Qualifications

• Nursing assistant diploma.
• Experience in similar work such as home care, psychiatry, dementia, or LSS.
• Preferably previous experience as a coordinator/planner.
• It is an advantage if you have worked with Lifecare and My Plans.
• You have a good command of the Swedish language, both spoken and written.
• For work in Home Care for Younger People, you need to be accustomed to cycling and have a manual driving license.

You are responsible, engaged, flexible, and attentive - with clients, relatives, and colleagues. You understand how important it is for each individual to be involved and have influence over their daily lives.

We place great importance on personal suitability. It is important that you maintain calm and focus, even in situations when a lot is happening at the same time or during rapid changes. You need to be confident in yourself and able to set boundaries when necessary.

About the Workplace

Home Care for Younger People has its gathering place at Bankgatan in central Lund. From here, we cycle or drive out to clients living around Lund city, distributed over four areas: Linero/Tuna, Centrum/Klostergården, Värpinge, Gunnesbo/Nöbbelöv, and Fäladen.

The home care of Lund Municipality is divided into several geographical areas. In each area, nursing assistants and care assistants work in teams with health care staff under the leadership of a unit manager. Each area has a unit administrator and coordinator/planner. We provide service, care, and home health care services around the clock to the municipality's clients.
