We are now looking for a Rehabilitation Coordinator for a temporary position.

As a Rehabilitation Coordinator, you will work with the psychiatric clinics in Northern Stockholm located at Alviks Torg and Alviks Strand. There are good public transport options just around the corner, such as the subway, tram, and buses. Alvik also offers proximity to Lake Mälaren, a good selection of lunch restaurants, and services. All employees have high competence and solid experience. There is a good atmosphere, low staff turnover, and everyone has the mindset to help each other.

Responsibilities

In the role of Rehabilitation Coordinator, you primarily work as personal support for the patient before and during work rehabilitation. The support includes mapping, assessment, and the establishment of rehabilitation plans as well as support in the implementation of the plan.

You will work closely with doctors and other therapists at the clinics. The work involves coordinating contacts between healthcare and other actors such as the Social Insurance Agency, the Employment Agency, the municipal district administration, or employers. The job also involves providing consultative support regarding sick leave and work rehabilitation to doctors and other professional groups at the clinics.

The assignment also includes participating in a so-called FINSAM team, which is a collaboration funded by the Coordination Association.

We would like you to have an interest in and knowledge of contributing to increasing the knowledge among employees about the sick leave process and various ways to return to working life after often long sick leave. Collaboration with other actors is an important part of the assignment.

Qualifications

Licensed healthcare professional (Licensed nurse, licensed physiotherapist, licensed occupational therapist) or authorized social worker.

You should have several years of experience in psychiatric care and collaboration with other actors. You should have MI competence and knowledge in insurance medicine.

You have good collaboration skills and the ability to work flexibly. You are solution-focused, attentive, and reflective.

Temporary position for one year with the possibility of extension. Full-time.
