Centralsjukhuset Kristianstad (CSK) is one of four acute hospitals in Region Skåne, employing around 2600 staff. We work closely with our neighboring hospital in Hässleholm. At CSK, most medical subspecialties are represented, including sections in cardiology, pulmonology, allergology, gastroenterology, hematology, rheumatology, endocrinology, nephrology, neurology, and stroke care, as well as surgery and orthopedics. Hässleholm Hospital has a leading elective orthopedics department.

The Medication Unit works for both hospitals in northeastern Skåne to improve patients' medication use and support healthcare operations in matters related to medication safety. We strive to reduce medication-related errors and minimize medication risks for patients in clinical practice. Our unit also has responsibilities that include medication supply, medication management, and educational initiatives in the field of pharmaceuticals within the operations. Currently, the Medication Unit consists of 25 employees, including seven clinical pharmacists.

RESPONSIBILITIES

As a clinical pharmacist with us, you will primarily work on the wards with medication reconciliations and interdisciplinary, in-depth medication reviews, mainly for elderly patients where medication-related problems are identified and then discussed with the responsible physician. The tasks offer great variation as they can be conducted within various specialties such as medicine, neurology, cardiology, infection, surgery, and orthopedics. The position includes performing medication reconciliations in the emergency department and on the wards, as well as interviewing patients and relatives to obtain an accurate medication history. As a clinical pharmacist, you also support the healthcare staff in other matters related to medications, including initiation and discontinuation, dosing, and administration.

The collaboration with healthcare personnel works very well, and clinical pharmacists are a sought-after competence. We have a shared phone number that healthcare staff can call to ask questions about medications and request assistance with medication reconciliation and reviews. This allows us to collaborate with many different doctors and wards. We also have a close cooperation with the unit's departmental pharmacists.

In addition to medication reviews, there may also be opportunities for quality work within medication safety to support the operations. Information and educational activities are regularly conducted for healthcare staff, and you may be involved in this. We are also a natural partner to the management's chief physician, chief nurse, and medication responsible physicians in matters regarding medications and patient safety.

In 2026, there is a preliminary transition to a new digital care system, SDV, where pharmacists will have important tasks in ensuring safe medication use for patients visiting inpatient care.

QUALIFICATIONS

We are looking for someone who is a licensed pharmacist or a licensed pharmacy technician with a master's degree in pharmacy. Additionally, you must have a master's degree in clinical pharmacy or equivalent experience working with clinical pharmacy that we assess as comparable.

You need to have language skills in Swedish at least at level C2 according to the Council of Europe's language scale, as well as good computer skills and proficiency in English. Practical experience with medication reviews and healthcare-related work is an advantage. If you are already familiar with Region Skåne's work regarding medication recommendations, we also consider this an advantage.

You need to have a good ability to collaborate in a team with various professional groups while also being able to work independently. Taking responsibility and making decisions with good judgment are important qualities for this position. We see that you have an easy time establishing contacts and trust among colleagues, supervisors, and internal and regional partners in the field of pharmaceuticals. Furthermore, you are a good communicator who conveys your knowledge in a pedagogical manner, both in writing and verbally. You have a knack for driving work forward and are goal- and results-oriented. Personal suitability will be given great importance.
