Social Worker and Discharge Coordinator, General Psychiatry Department 2

Job Responsibilities

The position involves working both as a social worker and discharge coordinator in the department. Your target group consists of patients and relatives, where you coordinate efforts with various societal actors, monitor and meet the patient's needs and rights. Your tasks include patient conversations, providing social counseling, and conducting social measures and investigations. You coordinate, support, and assist the patient in matters such as finances, employment, and housing, and provide support to relatives and close ones as needed.

Support is provided, for example, in the form of motivational conversations and suicide prevention discussions, based on needs until contact is established elsewhere. You should also be able to perform acute assessments and suicide risk assessments as needed. Together with the children's representative, you will highlight the needs of relatives' and close ones' children for advice, support, and information, and you should also be able to file a concern report if a child is at risk. Upon discharge from inpatient care, it is ensured that the patient can leave the care department with a safe and secure homecoming of good quality, where the patient is involved in this process. High demands are placed on flexibility to ensure that the patient receives an individual and tailored discharge process.

You will have overall responsibility for the discharge process, working according to a standardized method in accordance with the ViSam model, which is a collaboration model for planning and information transfer. Therefore, it is important that you can gather relevant information to continue planning the patient's care and treatment. This also means that you will have contact with care neighbors and collaboration partners.

In your role, you will participate in rounds and have daily check-ins with the care team, where you, together with the care team, are responsible for implementing care plans. After the care period, you will write and ensure that the patient has a care summary. Before the patient's homecoming, you are responsible for ensuring that the correct documentation, aids, and medications accompany the next caregiver.

Qualifications

Licensed healthcare social worker or a degree in social work, preferably with experience in psychiatry. You enjoy working closely with patients and always have the patient's best interests in mind. The work places high demands on your ability to communicate and collaborate with others, as well as express yourself well in speech and writing.

We place great importance on the ability to work independently, have an open mind, see opportunities, and take initiative. Your commitment and attitude contribute to a good work climate, where we all help each other and collaborate to achieve the best treatment quality for our patients. You are curious and open to trying new ways of thinking and methods and enjoy working in a team. Experience as a social worker in healthcare is an advantage. Personal suitability is highly valued.
