Close Care is the care that takes place close to the patient, primarily in the patient's home, and the Community Care Team based at Angered Local Hospital is part of this.

We now need a nurse for our Community Care Team with appropriate training in caring for the elderly at home.

We work to provide security for our most ill elderly by bridging inpatient care with primary care and municipal health and medical care. We work according to the patient's needs with the aim that they can remain in their home environment. We deal with acute changes and long-term planning. We work with a person-centered approach, and the patient is connected to the team based on the need for support and care from the Community Care Team, ranging from days to a month.

We also have a close collaboration with other community care teams within the Hospitals in the West and Sahlgrenska University Hospital, where we develop together, share knowledge, experiences, support each other, and shape the future of care.

Responsibilities

The Community Care Team consists of doctors and nurses. As a nurse in the Community Care Team, you will have a varied job where you work in a team but also independently. You will work according to treatment plans developed in the team. Assignments come from inpatient care, emergency or outpatient clinics, ambulance, or primary care. It is a varied job where you provide treatment, take samples, and follow up in the patient's home. You have a coordinating role around the patient, their relatives, and partners based on the patient's needs.

We are in constant development, and you are expected to participate in continuous improvements.

Qualifications

Licensed nurse with experience working with elderly patients with multiple diagnoses. A driving license (B) is required.

Conditions

Temporary position 40% until 2025-12-31 with the possibility of extension. We work weekdays during the day.
