To meet the residents' needs for more accessible care, Region Skåne has decided to collaborate with healthcare providers to start a new form of care in Skåne. A mobile team that strengthens existing healthcare providers and offers safe and accessible care for patients who prefer healthcare at home instead of in the hospital. The team acts as a coordinating hub and can assess, plan, prescribe, and perform interventions at home. Care is provided based on each individual patient's needs with the goal of offering qualified care both medically and psychosocially. We have been operational since January 2023, and the team consists of doctors, nurses, social workers, and medical secretaries. We physically operate from our own premises at the Hospital in Landskrona.

JOB RESPONSIBILITIES

We have received an expanded assignment that means we will work from September 2025 between 08:00 and 17:00 every day of the week. For nurses, this means working every sixth weekend and one to two light holiday days per year.

You will be one of four nurses in the team, actively participating in planning around patients with other professionals and collaborating with other healthcare providers. You often form a team with one of the unit's doctors but also work independently and make your own planned visits to patients, such as blood transfusions, antibiotic injections, wound dressings, and catheter changes. QUALIFICATIONS

We welcome you as a licensed nurse with the authority to practice within Swedish healthcare. Proficiency in Swedish at least at level C1 according to the Council of Europe's scale is required. Since we provide care in the patient's home or in special housing, a valid B driver's license is also required. We prefer that you have several years of experience working in somatic care.

You have good professional skills, and since your work is largely conducted in collaboration with both colleagues and other healthcare providers, it is a prerequisite for the position that you enjoy teamwork and have good collaboration skills. You are confident in driving your work independently, of course with the patient and relatives at the center. As a person, you have a flexible working style, are attentive, and take initiative. The management of Close Care and Health has existed since January 1, 2025. The management is responsible for grant-funded healthcare activities within close care. This includes, among other things, mobile teams and specialized palliative care, outpatient care within close care, acute care within close care, 1177 by phone, and certain region-wide functions. Additionally, the management is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is expected to be around 1,000.
