Welcome to us!

We are looking for an Assistance Case Worker for our client unit for the elderly. Currently, 14 dedicated case workers, a team leader, and a unit manager work here, together creating a supportive and developmental work environment. Every employee is an important part of our team, where we support each other in daily work and drive the unit's development forward.

We process applications according to the Social Services Act (SoL), the Act on Support and Service for Persons with Certain Functional Impairments (LSS), and the National Transport Service for Persons with Reduced Mobility Act. Our administrative department assists with invoice review and fee management, which gives us the opportunity to focus on social work and the core of assistance case work.

Our office is located in Skärholmen Center, close to shopping and a wide range of lunch options. We sit together with the remaining units within social services in an open office landscape.

Your role

As an Assistance Case Worker with us, you meet people over 65 years old with varying backgrounds and needs, including those with substance abuse issues, mental health challenges, dementia, or physical care needs. You are responsible for investigating needs, making decisions on interventions, and collaborating with both internal and external actors to find the best solutions for our elderly residents. Home visits are also part of the role.

Your main tasks include:

  • Conducting home visits and discharge planning from inpatient care
  • Working in the city's social systems, such as Paraplyet and Lifecare
  • Investigating and deciding on interventions such as home help services, day activities, and special housing forms according to the Social Services Act
  • Written documentation and case processing

Your skills and experience

We are looking for you who have a degree in social work or another educational background that the employer deems equivalent, although it is meritorious if you have a social work degree. It is also meritorious if you have experience working with public authority tasks within elderly care in Stockholm City, experience with the city's documentation systems Paraplyet and Lifecare, and good knowledge of relevant legislation.

You also need to be confident in standing in the center and sharing your knowledge with your colleagues as part of our operational development.
