Health and Support Coordination in Halland

In Halland, the municipalities and Psychiatry Halland collaborate on issues regarding health and support coordination. The work is based on national care and intervention programs (VIP) for ADHD, harmful use and addiction, as well as schizophrenia and similar conditions.

The purpose of health and support coordination in Halland is to improve the living situation for the individual and to create improved collaboration between psychiatry, social services, and other actors. In this way, health and support coordination is both a collaboration model and a working method.

Job Description

Health and support coordination in Laholm is a pioneer in interventions without needs assessment and has been offered for several years as a low-threshold activity without the need for a decision on assistance.

The intervention is offered to people with special needs for coordinated interventions over a longer period due to a mental disability. Especially prioritized in the county-wide agreement are people with psychosis-like conditions and comorbidity, but also people with a large care need and those who for various reasons need better coordinated interventions, regardless of diagnosis, are offered support.

We work in close cooperation with social services and healthcare, but on behalf of the client. Our keywords are Will, Choose, Acquire, and Keep, and our focus is on long-term sustainable changes. Preventive work also plays a major role through counseling and point interventions for former clients to prevent relapse. As a health and support coordinator with us, you are not only part of a team that inspires and mentors each other in everyday life, but also part of the cross-municipal networks in Halland.

Responsibilities

Health and support coordinators work based on an evidence-based method with a clear process. Together with the client, we conduct a mapping that leads to a change plan. We support contact with other actors and coordinate all interventions. If necessary, we create and work with resource groups based on the client's needs and wishes.

We also have a clear focus on recovery and use recommended methods and approaches for, among other things:

• Problem solving
• Preventing crises and relapses
• Stress management
• Social skills training

Are you the one we are looking for?

To work as a health and support coordinator, a combination of relevant education, practical experience, theoretical knowledge about mental health and social work, and the ability to coordinate interventions between different actors is required.

You who apply must have at least a post-secondary education in social work or alternatively a behavioral science education with experience of the target groups.

Work experience with the target groups is a requirement. Similarly, experience of leading SIP meetings and coordinating interventions, as well as good knowledge of society's authorities and their functions, is required. You command Swedish fluently in both speech and writing.

We would like you to have one or more of the following educations: Health and support coordination/Case management, supported employment/training/education, IPS, ESL, and MI.

As a person, you are good at creating contacts and maintaining professional relationships. You can lead and coordinate groups in a situation-adapted way. You take great personal responsibility for your work and plan, organize, and prioritize the work in an effective way. You have a high degree of self-awareness, are open to others' ideas, and dare to try new things.

A Category B driver's license is a requirement.
