Case Management Unit

 

The Case Management Unit within the Care Administration in Smedjebacken is a department consisting of 9 employees. This includes five case workers for Social Services (SoL)/Act concerning Support and Service for Persons with Certain Functional Impairments (LSS), two fee administrators/economic administrators, a health promoter, and a support pedagogue/relative supporter. The unit is led by a Head of Authority.

We are now looking for an additional case worker. Our target groups are growing, and so are we.

Responsibilities

As a case worker for SoL/LSS, your duties include:

  • Investigating and assessing needs, following up on measures, and sharing responsibility with colleagues for legal certainty.
  • Collaborating closely with key persons in individuals' networks, with a focus on ensuring that the individual has a good and secure life.
  • Cooperating with both internal and external stakeholders.
  • The role involves both authority exercises and non-means-tested measures according to the new Social Services Act.

As a case worker with us, you will follow the case from the initial report/application, through investigation, to implementation. The content and conditions of the work may change over time based on new requirements for accessibility and preventive measures in the new Social Services Act.

Qualifications

Requirements:

  • A degree in Social Work or another equivalent higher education degree in a relevant field.
  • Good ability to express yourself in Swedish, both orally and in writing.
  • You must be well-versed in the case handling requirements and legislation governing the operation.
  • Good computer skills and a valid driver's license (Category B).

Merits:

  • Experience with case handling and investigation using IBIC (Individual's Needs in Focus).
  • Knowledge of the activity system Life Care.
  • Cosmiq Link, the discharge process from inpatient care.
  • Previous experience in social work and authority exercises.
