About the roleWe are now looking for a Health and Social Care Counselor for the Emergency Department, Medicine, Geriatrics, and Rehabilitation – Avesta Hospital. Do you want to work closely with patients and make a difference in people's lives? Here, as a Health and Social Care Counselor, you play a vital role in a multidisciplinary team where a holistic approach, collaboration, and the patient's own resources are in focus.
The Paramedicine Unit, where the position is organized, collaborates with the entire Department of Emergency Medicine, Geriatrics, and Rehabilitation at Avesta Hospital. The department has a total of approximately 200 employees, and within the Paramedicine Unit, there are 18 staff members. The department includes both outpatient clinics and inpatient wards. As a counselor here, you will have a varied and developing assignment with both breadth and depth. Your main placement is within the outpatient team for geriatrics and rehabilitation, where you work together with occupational therapists, physiotherapists, doctors, nurses, and assistant nurses.
The team meets patients with neurological diseases and injuries, where you are responsible for the psychosocial assessment and treatment work in close collaboration with other professions.
ResponsibilitiesAt our department, you will be involved in several parts of the clinical operations, which provides a broad and stimulating assignment:
- Psychosocial investigation and treatment work in a team
- Support for patients and relatives
- Collaboration with external actors and networks around the patient
- Participation in the heart team within the medicine area
- Assessment and treatment of patients in the medicine clinic
- Interventions for inpatients, for example on the stroke ward
You work from a holistic view of the patient, where we together strengthen the patient's own resources and participation.

QualificationsWe are looking for you who:
- are a licensed Health and Social Care Counselor
- have a driver's license for a manual transmission car (home visits may occur in the work)
- have an interest in geriatrics, rehabilitation, and medicine
- have good ability to work both independently and in a team
- are flexible and structured
- have good knowledge of society's support measures for individuals and their networks
- thrive in a role where you contribute your specialist competence
- can lead groups and collaboration meetings
Meritorious:
- Experience in neurology, rehabilitation, or work in multidisciplinary teams
