Our 1,800 employees work with primary care and health in Uppsala County. We offer a wide range of services within primary care. Our patients meet us at Region Uppsala's own health centers or within habilitation services, and we provide advice by phone via 1177. Education, research, and development are important to us, as we tailor our offerings based on the needs of the residents. We already offer mobile teams, video visits, and smart e-services.
We are now looking for a new colleague for the primary care ward in Tierp. Start date is by agreement, and the position is 70–100%.
Here, you work closely with a care team consisting of nurses, assistant nurses, physiotherapists, occupational therapists, pharmacists, and dietitians as needed. The work involves assessing patients for potential inpatient care on the ward, managing inpatient care, and planning for discharge. We place great emphasis on individualizing care and involving both patients and relatives, as well as planning ahead together with the health center and municipality to create security and clarity after discharge.
Currently, two specialists work part-time at the unit, and we are now expanding with a specialist colleague with the goal of always having two doctors on duty who share the medical responsibilities on the ward. At our workplace, you work weekdays between 08:00–16:30. Compensation is provided related to the geographical location.
On-call duties are not included in the role.
The primary care ward is part of Tierp Health Center. The ward has 18 care beds with shorter lengths of stay and adapted admission and discharge flows. The assignment is at the primary care level for patients aged 17 and above. The ward primarily cares for older patients with complex needs who require inpatient medical care but are not assessed to benefit from hospital resources.
Patients are admitted from health centers, the municipality, the emergency department, mobile primary care teams, and Akademiska Hospital. The ward also has some day-care activities.
We work from a patient-centered approach, placing great importance on involving the patient and relatives in the care.
We are looking for you who are a specialist in general medicine, internal medicine, or geriatrics and hold a Swedish medical license. You have a commitment to meeting patients with increasing frailty and adapting care based on the individual's wishes and conditions. You are accustomed to working independently and taking initiative but are also interested in a team-based approach.
It is important that you are a good communicator in Swedish, both orally and in writing. You have good collaboration skills and appreciate teamwork. Since the primary care ward is a relatively new form of care, we are keen for you to be interested in developing the operations together with other employees.
