Vårdcentralen Centrum is an established practice with approximately 13,000 registered patients, making us one of the larger primary care centers in Skåne. Here, we have a combined and diverse competence including nurses, doctors, psychosocial specialists, and rehabilitation staff. RESPONSIBILITIES

As a Rehabilitation Coordinator, you will have a central function in supporting patients during sick leave and rehabilitation. This involves coordinating contacts between the patient, care team, employer, authorities, and other actors to create efficient and sustainable rehabilitation processes. The work is done in close collaboration with doctors, the psychosocial team, physiotherapists, occupational therapists, and other professions at the primary care center. Your duties include coordinating rehabilitation efforts for sick-listed patients, providing support throughout the entire rehabilitation process, and collaborating with employers, the Swedish Social Insurance Agency (Försäkringskassan), the Public Employment Service (Arbetsförmedlingen), and other external actors. You will also contribute to quality and structure in sick leave and rehabilitation work, participate in teamwork and the development of the primary care center's rehabilitation processes, and support healthcare professionals in matters related to rehabilitation and sick leave.

The position implies that you are employed in your core profession as a nurse, occupational therapist, physiotherapist, social worker, or social worker (counselor). You will work full-time with the assignment as a Rehabilitation Coordinator, although this may change over time.

QUALIFICATIONS

We are looking for you who are a licensed nurse, occupational therapist, or social worker (counselor) with the eligibility to work within Swedish healthcare. You have good language skills in Swedish, corresponding to at least level C1 according to the Council of Europe's scale. Alternatively, a trained social worker with good Swedish language skills. Additionally, you have previous experience of rehabilitation work and collaboration. Experience from primary care and/or a coordinating role is meritorious and provides a stable foundation for quickly entering the assignment and developing further together with the practice. If you have training as a Rehabilitation Coordinator, this is also meritorious.

For the position, it is important that you have the drive to take initiative and lead processes forward, as well as the ability to create structure and progress in complex cases. We want you to have good communication skills and a secure way of meeting people to build good relationships both internally and externally, as well as contributing to creating trust and positive results.
