Mobile Healthcare Team Seeks District Nurse in Gävle

The Mobile Healthcare Team provides home care for patients who have difficulty reaching their regular healthcare provider. The team is organizationally part of VO Primary Care in Gävleborg County. We work broadly and collaborate with all health centers in the area, regardless of ownership, as well as with municipal healthcare and the operations at Gävle Hospital. Doctors and nurses work together in the team to ensure professional and secure care. The operation is based on a team vehicle that drives through Gävle Monday to Friday to enable close and coordinated care interventions for our patients.

Job Responsibilities

The work of the Mobile Healthcare Team is based on a holistic assessment of the patient's situation in their home environment. Through team assessment and coordination of resources, it is ensured that the patient receives the right interventions at the right time. The work involves, among other things, contact with patients, relatives, and other healthcare providers to plan home visits and handle incoming patient cases.

As a District Nurse with us, you will:

• Receive and distribute incoming patient cases.
• Plan and book home visits.
• Collaborate with various healthcare actors.
• Work in a team during assessments and treatment of patients.
• Make your own assessments and treatments of patients with support from doctors.

Qualifications

We are looking for you who are proactive and confident in your professional role, with the ability to independently plan and prioritize work based on patient needs. You are flexible and adapt easily when conditions change in daily work. Since the work is based on close collaboration with both colleagues and other actors within healthcare, we see that you have good teamwork skills and contribute to a positive and professional work climate. You see opportunities in changes and are eager to contribute to developing working methods that strengthen care for our patients.

In the role of District Nurse, you must:

• Be a licensed District Nurse.
• Have very good knowledge of Swedish, both spoken and written.
• Hold a valid Category B driver's license.
