Job Description
Open Care Housing is a unit within the Employment and Social Services Administration that offers support to citizens in Malmö who need assistance before, during, and after obtaining housing. "Before" may involve advice and support in securing a home or private apartment. "During" may involve maintaining one's housing, working towards a private contract, or working towards other forms of housing. "After" may involve ensuring that everything is in place after securing a private home/contract to avoid missing the first rent payment, getting a guardian started, etc. Open Care Housing also works with the rehabilitation model "Case Management Strength Model." This intervention targets residents of Malmö who have difficulty managing daily life (both in the home and in society) and need support to work long-term on factors that may contribute to problems. In Case Management, outreach work is the starting point, meaning that the work is largely conducted outside of an office environment.
Together with the resident, you set goals and plan the intervention in an implementation plan/strength analysis that is regularly followed up. You work in a coordinating role and assist with the contacts, help, and collaboration that the resident desires and consents to. The aim is for the resident to reach their goals and, in the long run, live an independent life.
The tasks vary from mapping conversations and support conversations to collaboration with other professional actors, such as investigating social secretaries and other actors within Social Services, healthcare, the Swedish Public Employment Service, and other executing units within the Employment and Social Services Administration.
Furthermore, the work will also include administrative tasks such as statistics, follow-up, evaluation, developing routines, and documentation.
At Open Care Housing, there are interventions for people of different ages and with different life histories. This requires the ability to switch between different strategies and approaches depending on the person you are facing. It also requires that the unit constantly evaluates and finds new solutions.
In our unit, you won't hear "because we've always done it that way" when you question something. Instead, we discuss based on the purpose of our work, and if you have a better solution, we listen to you. We draw inspiration from other units and prioritize time for training and study visits.
Qualifications
We are looking for you with a degree in Social Work (Social Work) and an interest and commitment to homelessness. You have experience working with coordination for people with long-term exclusion and/or co-morbidity. If you have previous experience working with homelessness, harmful substance use and addiction, mental illness, follow-up and evaluation, as well as in-depth knowledge regarding drug use and mental illness, open care, or residential treatment, it is meritorious. You express yourself well in Swedish, both orally and in writing, as well as in English orally. Knowledge of additional languages and a B-driving license is meritorious.

The person we are looking for is creative and solution-focused with a desire to influence and develop the unit. For us, it is important that you can work according to requirements and guidelines but at the same time maintain a meta-perspective and see how the unit can be changed in the long run. You have a low-affective approach and have the ability to see strengths and resources in the people you meet in your work. You are unpretentious, have the ability for self-insight, and have a well-developed ability to collaborate as well as the ability to structure, plan, and drive your work forward yourself.

Open Care Housing consists of three sections with 14 employees and a section manager per section.
