Do you want an independent and varied job where you make a difference for patients' health and opportunities to return to work? We are now looking for a healthcare social worker who also wants to work with rehabilitation coordination at our rehabilitation clinic at Högsbo Primary Care Hospital.

About the position

As a healthcare social worker with us, you work with psychosocial support for patients within various medical fields. The role also includes duties as a rehabilitation coordinator, where you actively work to facilitate patients' return to work through coordination, planning, and support in the rehabilitation process.

The rehabilitation clinic collaborates with units within the hospital, such as the heart and diabetes clinic, neurology clinic, gynecology clinic, and other specialist clinics. The work involves a lot of collaboration but also offers great opportunities for independence and flexibility in your working day.

Your responsibilities

• Provide psychosocial support and treatment to patients and their relatives
• Assess and work with rehabilitation needs from a holistic perspective
• Rehabilitation coordination with a focus on supporting patients' return to work
• Collaborate with care teams, employers, the Swedish Social Insurance Agency, and other actors
• Participate in the development of working methods and collaboration models
• Provide support to other units in development work, such as domestic violence

Qualifications

• Degree in Social Work (Social Work degree)
• Registered Healthcare Social Worker
• Experience working as a social worker within healthcare
• Good ability to work independently and in a structured manner

Merits
