SiS LVM-hem Hornö receives men with alcohol, drug, or combined substance abuse. We admit men with mental health issues and men with acting-out behaviors. Our mandate is to provide care for clients with special care needs.

Job Description

A nurse's education in the field of nursing contributes specific competence to prevent and improve, as well as restore and preserve, the health of those cared for at SiS. The nurse is to provide good health care with good nursing care, which includes planning nursing care, implementing and following up on nursing measures, and being responsible for and leading the nursing work.

The nurse's main areas of responsibility and work within SiS are medication management, health assessments, and nursing interventions, as well as collaboration with both internal and external parties regarding health care in connection with placement, during the care period, and prior to discharge from SiS.

Key responsibilities for a nurse at SiS:

• Responsibility for medication management, including ordering, preparation, and administration of medications, as well as follow-up and evaluation of the patient's prescribed medications.
• Responsibility for checking and handling medication and signature lists.
• Training staff in safe medication management and delegating tasks related to parts of medication management to relevant staff at the institution, while following up on the delegation.
• Participate in treatment teams relevant to the institution and contribute health care expertise to treatment planning.
• Assess and plan for the patient's care needs, and handle relevant contacts with regional health care units in the patient's care chain.
• Assess and perform nursing interventions and measures for emerging health problems in daily institutional care.
• Conduct health assessments, including planning and following up on interventions both internally and in the external care chain.
• Prepare incoming questions for medical rounds, collaborate with doctors during rounds, and assist in follow-up work, such as following up on planned and implemented interventions.
• Contribute, together with the institution's other health care unit, to the systematic quality and improvement work of health care at the institution.
• Conduct educational initiatives within the nursing field and health care field for the institution's employees.

Competency Requirements

Category B driver's license

Education

Registered Nurse (RN).

Knowledge and Skills

• Experience working with people in vulnerable life situations, for example, work within addiction care or psychiatry.
• Experience in work where assessments and interventions are made for various mental and somatic conditions, for example, within primary care or emergency care.
• Good ability to express oneself in Swedish, both written and orally.

Employment:

The position is a permanent full-time employment starting with a six-month probationary period. Start date by agreement.
