Job Description
Do you want to work close to operations and make a difference in the daily lives of both residents and staff? Are you an experienced nurse looking for an advisory, supportive, and developmental role? Then you might be the person we are looking for in our new project in Säffle Municipality.
About the Project
Säffle Municipality provides care and social services with a focus on safety, quality, and good collaboration. As an advisory nurse, you play a key role in supporting home care staff with health and medical-related questions. The assignment is primarily advisory and contributes to strengthening both patient safety and staff competence, as well as participating in coordination related to the discharge process.
The position is during daytime hours.
The project has an initial duration of 1 year, with possible extension after evaluation.
Responsibilities
In the role of advisory nurse with coordination responsibilities, you will among other things:
- Provide advice and support to home care staff regarding care and health issues.
- Assess health conditions via telephone. Access to digital assessment tools is available.
- Participate in discharge processes, utilizing nursing expertise.
Qualifications
- Registered Nurse (RN).
- Experience in healthcare, preferably within discharge processes and advisory roles.
- Confident in your professional role and accustomed to making independent assessments.
- Good pedagogical and communicative skills.
- Good command of Swedish, both spoken and written.

You have a professional and calm approach, are solution-oriented, and collaborative. You enjoy supporting others and contribute to a positive work climate.
