Description

We are looking for a nurse for the Municipal Primary Care unit to work within the special housing team. You will be part of teams with other nurses, but also in a cross-professional team with occupational therapists, physiotherapists, unit managers, support staff, and other professions surrounding the patient, with a focus on patient safety and person-centered care. You have great opportunities to plan and structure your workday so that it works for you, your patients, and your colleagues. You will be the responsible nurse for care at one of our central housing facilities in Sunne.

Our housing facilities are centrally located within walking and cycling distance, but travel by car is otherwise included.

You work mostly during daytime on weekdays and approximately every fifth weekend, with flexible working hours. We offer individually adapted introduction, a mentor, and collegial supervision.

Job Responsibilities

Your responsibilities include coordinating and ensuring that specific nursing interventions are planned, implemented, and evaluated, as well as training, supervising, and delegating tasks to support staff. You perform medical assessments, specific nursing interventions, and work in a health-promoting and preventive manner.

The work involves planning and following up on nursing interventions independently or together with colleagues. Operational systems that support us in our work include digital signature lists and the medical record system Life Care HSL.

Qualifications

We are looking for you who are a licensed nurse with commitment and interest in municipal primary care with a focus on the elderly and dementia. It is an advantage if you have worked with similar tasks previously. You are welcome to have specialized education with a focus on district nursing, elderly care, or psychiatry.

To succeed in the position, you work independently and take significant responsibility in your profession as well as for the planning and follow-up of healthcare.

You value teamwork highly and have a strong interest in working with frail elderly, dementia, and palliative care. As a person, you are stable and flexible, have a high sense of responsibility, and have the ability to work both independently and in a team. You are driven by challenges and want to develop both within your profession and within municipal primary care. We place great emphasis on personal suitability with the ability to work with different focuses and in different groups based on the needs of the operations.

Conditions

• Swedish license as a nurse and several years of experience working as a nurse.
• A B-driver's license (manual transmission) is a requirement, as well as the ability to cycle.
• Two reference persons, at least one of whom is a manager, to be provided at the interview.
• In connection with possible employment, we request extracts from the Police's criminal and suspicion register. A record may affect your opportunity for employment.
