Nyhälsan is part of Bräcke Diakoni, an ideologically driven foundation where care is developed close to patients' needs, with employees' knowledge and engagement as a key component. In this role, you as a specialist physician will combine the general medical duties at the health center with special responsibility for patients living in ordinary housing. This is a role for you who want to use your medical judgment and see your values come into play in everyday life.

Your Role

As a specialist physician at Nyhälsan, you have a shared role between the health center and home care, with approximately two days per week in home care and three days at the health center in Nässjö and Forserum. In home care, you primarily meet elderly and multi-morbid patients whom you follow over time. Here, medical assessments, care planning, and collaboration are important parts of working preventively at the right time.

You have significant influence over planning, scheduling, and patient responsibility within the scope of the role. Since the operations are developing new working methods, you will also contribute to how the physician role in home care and team-based work should function in practice.

Who We Are Looking For

We are looking for you who are a specialist in general practice or geriatrics and have experience in primary care. You want a varied role where you move between the clinic and the patient's home, and where your medical competence is used close to the needs found in everyday life. You are confident in your medical judgment and thrive in collaboration with other professions, both regarding patients and in the development of working methods.

Qualifications:

• Licensed physician with Swedish authorization
• Specialist competence in general practice or geriatrics
• Experience in primary care work
• Experience working with elderly and multi-morbid patients
• Valid driver's license (Category B)
• Previous experience in home care is an advantage
