Acute Specialist Care at Home – The Akademiska Hemma Department.

Akademiska Sjukhuset is entering an exciting phase of development where we are looking for engaged employees with an interest in healthcare development and who are ready to think outside the box. Instead of the patient being admitted to a ward, healthcare professionals will visit the patient at home to provide the care typically performed in inpatient wards. The inspiration for this care model initially came from other countries where the "Hospital at Home" concept has been used successfully, but for the past few years, it has been implemented in several locations across Sweden with very high patient satisfaction and patient engagement.

Patient visits occur based on need and diagnosis, 1–3 times per day. The work will be led by a team center in collaboration with mobile teams that plan and prioritize both physical and digital visits. The mobile nursing team will consist of two people, and rounds will take place digitally via video calls with the team center nurse and the responsible physician.

The care model is in its startup phase and is continuously developed in collaboration with colleagues and the project group, meaning you will have the opportunity to influence working methods and routines. We have started with individual patients and plan to expand to a full-sized ward with just over 20 care beds.

Your Role

As a mobile nurse at the Akademiska Hemma Department, you will be responsible for the care of patients in their home environment. During home visits, tasks may include blood sampling, monitoring vital signs, intravenous antibiotics, subcutaneous injections, pain optimization, postoperative care, stoma training, wound dressing changes, fluid therapy, etc. Initially, you will work with standard nursing duties during home visits, and based on experience, you may also take on responsibilities for the team center. There is always a responsible physician at the hospital who, in close collaboration with the nurse at the team center, plans the patient's care.

Your Qualifications

We are looking for you who are a licensed nurse with an interest in this care model. Experience in inpatient care and/or home healthcare is meritorious. You should be flexible in your thinking and have an interest in this care model and the digitalization of healthcare. A valid driver's license (Category B) is required.

We Offer

We offer a permanent full-time position, with a start date to be agreed upon. Probationary employment may apply. The working hours are initially scheduled for days, evenings, and weekends, but during full-scale operations, night shifts may become available upon agreement. Employment is ongoing.

• An introduction consisting of both clinical and theoretical sessions, tailored individually over 2–5 weeks. Part of the introduction takes place in inpatient wards within surgery, urology, or infectious diseases, depending on previous experience in these areas.
• We apply healthy scheduling with a focus on recovery. The day shift starts at 07:30, and the evening shift ends at 21:30, including meal breaks.
• An inpatient care supplement of 2,000 SEK/month for a 100% position, in addition to the base salary.
• Opportunities for rotation positions within other inpatient wards based on your preferences.
