Description

Skaraborg Hospital's (SkaS) mobile Primary Care Team offers you, as a doctor, a unique opportunity to try out an exciting assignment. You combine your regular position with your new assignment by alternating between your regular unit and the Primary Care Team in a combined role. Service is performed at each unit for 1–3 weeks at a time. The setup can be adapted to your preferences. Since the assignment covers operations for the entire Skaraborg region, the team operates from both Skövde Hospital and Lidköping Hospital.

Regional routine framework for combined roles

SkaS's mobile Primary Care Team acts as the hospital's extended arm, working in patients' homes throughout Skaraborg. The Primary Care Team's assignments come from all SkaS care units and emergency departments, pre-hospital care, the Healthcare Dispatch Center, as well as regional and municipal primary care. The operation is assignment-based with the aim of increasing the conditions for specialized home care, reducing unplanned inpatient care and emergency visits, or replacing inpatient care.

As more care can be provided outside the hospital, we are now looking for you who want to help develop the Primary Care Team as an alternative form of care.

Responsibilities

In your role as a doctor in the Primary Care Team:

• You are responsible for planning medical care
• You assess incoming assignments and initiate home care that can replace or shorten inpatient care
• You perform home visits together with nurses or conduct digital care meetings
• You collaborate with colleagues, care partners, and other care units daily
• You are involved in and drive the development of the operations

You will work together with a well-functioning group of nurses from various hospital departments and the ambulance service. We work together to provide the patient with safe and secure care at home.

Qualifications

Swedish medical license with specialist competence.

Competence and experience in internal medicine, cardiology, infectious diseases, or geriatrics is an advantage. Other specialties are also welcome to apply as personal suitability is highly valued.

You are easy to work with, have good communication skills, and a holistic view with the patient in focus. We value the ability to collaborate well, as the work involves many contact points with care partners and colleagues.

You have an interest in participating in development work to improve care for the residents of Skaraborg.
