The Health and Elderly Care Administration is one of the largest departments in Borås Municipality. Professionalism, learning, and engagement are the guiding stars of our work.

The department's authority activities are responsible for handling measures according to the Social Services Act, as well as housing planning, home adaptations, fee handling, and care planning. The authority activities employ approximately 80 staff members, of whom about 50 are social workers. The management consists of a department head and four unit managers. The Planning Team is responsible for all admissions to municipal home healthcare, both regarding admissions from regional primary care and the planning of healthcare and rehabilitation measures in the patient's home following a hospital stay. There is a well-established collaboration between the team and the authority's social workers to ensure that support and assistance are designed in the best possible way based on the individual's own needs and resources. Our office premises are located at Ramnåsgatan 1, close to the Central Station and Travel Center. We work during the day, Monday to Friday.

Occupational Therapist for the Health and Elderly Care Administration's Planning Team

Job Responsibilities

One of your main tasks as an occupational therapist in the Planning Team is to carry out care and support planning. You gather information about the patient's needs and resources to plan their continued care needs when they are ready for discharge from the hospital.

During care and support planning, you collaborate with colleagues within the Authority and with hospital staff. Together, you contribute to making care transitions between inpatient and outpatient care safe for the patient. Collaboration and dialogue with rehabilitation clinics and outpatient clinics in connection with patients being admitted to municipal primary care is also a central part of your work.

The transition to "proximity care" places high demands on collaboration and the development of work processes. You and your colleagues in the Planning Team are responsible for driving, following up, and developing the working methods regarding planning and contacts with the Region's hospitals, primary care, and rehabilitation clinics. You actively work on developing our internal working methods and monitor other municipalities' working methods, not least the digitization of systems and assistive devices such as medication robots. It is also part of your assignment to introduce and train colleagues in the care planning process and in the IT support system SAMSA. You also serve as a knowledge resource for both internal and external collaboration partners, e.g., staff on hospital wards.

Qualifications

We are looking for a licensed occupational therapist with the right to practice within Swedish healthcare. You have experience from patient-centered work in the role of an occupational therapist within municipal primary care, inpatient care, and/or outpatient care.

You have a good ability to express yourself verbally and in writing, and your ability to be communicative, attentive, and solution-focused makes it easy for you to create and maintain good relationships. Having a good attitude is a given for you, regardless of whether you meet patients, relatives, colleagues, or collaboration partners. You have a very good ability to collaborate and cooperate in a trustworthy and professional manner with many different actors.

You enjoy working in a team and have a consultative working style. You need to be able to independently plan, structure, and prioritize your work, being meticulous yet efficient. We also expect you to be engaged, show initiative, take your own initiatives, and contribute to job satisfaction.

Good experience of working with various healthcare-related IT systems is a prerequisite for fulfilling the assignment. A B-class driver's license is required.
