Job Responsibilities
The Home Plan Team offers an intensive and flexible family treatment for families with children and adolescents who exhibit norm-breaking behavior. The target group is families with children between the ages of 10 and 18.
The Home Plan Team works with a target group of norm-breaking children and adolescents and their closest network. This may include children and adolescents with outwardly aggressive behavior, high conflict levels at home and elsewhere, associations with older criminals, or being seen in places associated with drug dealing, where there is a risk of developing criminal behavior/thought patterns.
The goal of the intervention is to help families find a functioning way to manage their difficulties and to develop effective communication pathways. The Home Plan Team works closely with Lyftet and Flexet, which are two other sections within the Platform unit. Lyftet or Flexet often take responsibility for the support and treatment of the child/adolescent, while the Home Plan Team focuses on treatment for parents and other important individuals in the child's/adolescent's life. The Home Plan Team primarily conducts treatment in the home. At least once per term, the Home Plan Team offers parenting courses according to Connect.
The work is scheduled from 08:00 to 16:30. There is an opportunity to work weekends. Documentation is done in Journal Digital.
Qualifications
Requirements:
- Degree in social work, behavioral science, social pedagogy, or another higher education qualification deemed equivalent by the employer
- Experience in treatment work with the target group of adolescents and their families
- Driver's license for a manual transmission vehicle
- Good knowledge of Swedish in both spoken and written form
Meritorious:
- Further education in treatment work or conversation methodology e.g., CBT, FFT, IHF, or MST
- Experience with families facing mental health issues, criminality, and neuropsychiatry
- Knowledge of other languages besides Swedish
As you meet many people daily, both adolescents and their families as well as colleagues and other stakeholders, you need to have a good ability to collaborate and create relationships. Since a large part of the job involves coordination and contact with multiple stakeholders, you need to be structured. It is also important that you are flexible regarding working hours, changing circumstances, and a willingness to try new working methods.

The Home Plan Team, together with Flexet, is a merged outpatient service within the Labor Market and Social Administration and is organized within IFO Insats, Platform Unit Malmö, support and treatment. The section consists of two section managers with shared leadership, two coordinators, nine family therapists, and fourteen youth therapists.
