Nurse at the Memory Clinic in Gävle

The Memory Clinic in Gävle offers specialist healthcare for the region's residents with cognitive diseases. Here, we receive patients who have been referred after a basic dementia investigation and conduct in-depth cognitive assessments as an important part of the diagnostic process. The clinic employs doctors, nurses, and occupational therapists who work closely together to provide patients with safe and professional care. As a nurse with us, you will be a central part of the team and contribute to the development of care for individuals with memory disorders.

In geriatrics, we are passionate about ensuring that older patients with multiple illnesses receive the best possible care. There are 16 care places, half of which are intended for frail elderly patients with acute illness and half for geriatric rehabilitation. The team includes nurses, doctors, nursing assistants, as well as physiotherapists and occupational therapists. Collaboration with other healthcare providers is a crucial task to help our patients achieve their best life. The mission of the geriatric department is to provide advanced medical and rehabilitative treatment to elderly patients in the region. Treatment is based on a geriatric holistic approach and person-centered care with the patient and their relatives at the center.

This role involves a rotation service, meaning you will spend 80% of your time at the Memory Clinic and 20% at the geriatric department.

JOB RESPONSIBILITIES

As a nurse at the Memory Clinic in Gävle, you will play a central role in the patient's care journey. You will meet both patients and their relatives during initial and follow-up visits, contributing your expertise to the assessment and follow-up work. The work is carried out in close collaboration with doctors, occupational therapists, and social workers, where the team's collective effort is crucial to providing patients with safe and professional care. You will also have an important responsibility for maintaining contact with relatives and external actors such as municipalities and primary care, as well as following up on medication treatments and participating in various medical procedures.

Your main responsibilities include:

  • Planning and inviting patients to initial and follow-up visits
  • Conducting family history interviews during initial visits
  • Participating in follow-up visits alongside a doctor or occupational therapist
  • Maintaining contact with patients, relatives, and external agencies (e.g., municipalities and primary care)
  • Being responsible for medication follow-ups and evaluating effects and possible side effects in consultation with a doctor
  • Performing blood sampling and assisting with lumbar punctures
  • Answering incoming calls via the TeleQ system.

QUALIFICATIONS

We are looking for a flexible and committed person who thrives in close collaboration with all professions in a varied everyday life. You have good collaboration skills, are confident in your professional role, have a genuine interest in working with individuals with cognitive impairments, and have the ability to meet people in a professional and empathetic manner.

In the role of nurse, you should:

  • Be a licensed nurse
  • Be able to communicate fluently in Swedish, both spoken and written, at least at C1 level.

It is an advantage if you have previous experience working with individuals with cognitive impairments as well as previous experience working with geriatric patients. Holding a B driver's license is a plus.
