JOB RESPONSIBILITIES

Do you enjoy performing medical tasks and providing care to patients in their homes? Many patients are unable to get to healthcare facilities on their own and need you as a nurse to come to them instead. This is to provide care based on the individual's needs. We are looking for inspired, engaged, and skilled nurses who want to work in home care. As a nurse in home care, you enable patients to receive medical care and nursing at home, allowing them to stay at home in a safe and dignified manner. We work in teams with doctors, physiotherapists, occupational therapists, home care providers, and other caregivers. Additionally, you will have a close dialogue with the patient and their relatives. All with the aim of providing good and close care. As a nurse in home care, you will have a central and important role for our enrolled patients.

Your responsibilities include:

• Performing health and medical care interventions for patients in home care.
• Being responsible for nursing and conducting medical assessments.
• Working in a team around the patient with, among others, physiotherapists, occupational therapists, and doctors.
• Planning and conducting home visits and follow-ups.
• Guiding and delegating nursing tasks to nursing staff and students.
• Documenting in Treserva and registering information in other operational systems.
• Working preventively to promote the patient's health and well-being.
• Working in our consulting operations during evenings and weekends.

The position primarily involves daytime work, but the schedule also includes evening and weekend shifts. You will then work in our consulting operations, which include the municipality's care and nursing homes, home care, as well as service and group homes within LSS and social psychiatry.

QUALIFICATIONS

We are looking for you who are a present and secure nurse who wants to work with home care. You are stable in your professional role, instill calm and trust, and see opportunities in changes while promoting development towards set goals.

• Licensed nurse
• Experience as a nurse
• Good knowledge of the Swedish language in speech, writing, and reading comprehension
• Driver's license (B) and experience driving a car

It is an advantage if you have experience in municipal health and medical care and if you are familiar with the systems Treserva, TES, Lifecare, NPÖ, and Pascal.
