Primary care in Region Jämtland Härjedalen is organizationally part of Division Close Care, together with Ambulance, Dental Care, Psychiatry, and Child and Adolescent Psychiatry.

Primary care is organized into 4 local care areas, and since January 2022, primary care and ambulance services have been integrated under good and close care. Each local care area is an economic unit led by a manager. Primary care is also responsible for the region's medical advice, youth reception, and the local care department Näva, all of which are grant-funded.

The Hammarstrand health center is part of the local care area of Berg, Bräcke, and Ragunda. The health centers in the area have a close collaboration. Hammarstrand health center collaborates extensively with Stugun health center. This position means that the work is linked to both Hammarstrand and Stugun, with a distribution of about 70%/30% per area.

JOB RESPONSIBILITIES

As our current district nurse will be going on parental leave for a year, we need a new nurse who can collaborate with the municipality and conduct home visits for individuals who are temporarily fragile due to illness. The individuals you will handle do not have home care but may have difficulty coming to the health center due to temporarily reduced ability following hospitalization or other illness.

You will have at your disposal a well-equipped car with medical equipment, which means you can perform many examinations at home that are normally conducted at the health center. You will also collaborate with home care groups to ensure that the individual has a comprehensive care situation based on needs. In this role, you will always have access to all resources available at the health center, which includes direct contact with doctors who can accompany you on home visits if necessary.

In this role, you have a great opportunity to develop the function as we have project funding for at least another year.

QUALIFICATIONS

• You must be a licensed nurse; if you are a district nurse, it is an advantage.
• You need to feel comfortable making home visits and conducting your own assessments.
• You must also have the ability to reprioritize as needed.
• It is an advantage if you have experience from both the municipality and the region.
• We place great emphasis on personal suitability.
• A valid B driver's license is required.
