Hospital at Home is part of Skåne University Hospital (Sus) - Malmö and is operated in collaboration between the emergency care and internal medicine departments as well as the infectious diseases department. This care model means that patients in need of acute specialist care, who wish to do so, can receive their care in their own home while maintaining medical quality and safety.

The model is based on the internationally established concept of Hospital at Home, which has been successfully used in several countries. Research shows that home care often leads to positive effects for patients, such as better sleep, improved appetite, increased activity, and reduced risk of confusion or falls. Our experiences so far confirm these results, and we see very high patient satisfaction and an increased degree of patient involvement.

At Hospital at Home, we bring hospital care to where the patient feels safest. We offer the same medical quality as inpatient care, but in an environment that promotes calm and recovery. Through digital tools, we maintain close contact and ensure that patients can always reach us, and when their condition requires it, we quickly arrange for hospital admission.

We are a warm and competent team where everyone contributes with their unique experience. Here, nurses, doctors, and rehabilitation professionals work together to create safe, secure, and personalized care for patients from many parts of the hospital. We are constantly evolving and are curious about new ways of working, and we hope you want to develop with us.

As a nurse in Hospital at Home, you will have a varied, independent, and meaningful everyday life. You will meet patients in their homes and work with both medical and nursing-related assessments and treatments, many of the interventions that otherwise take place in a hospital ward. You will be the patient's trusted contact person, and you will also support them in how our digital solutions work. At the same time, the team is always close by for support, brainstorming, and joint solutions. Here, we work with care, professionalism, and great pride. The work is scheduled for day and evening shifts, both weekdays and weekends, but you will not work nights with us.

QUALIFICATIONS

We are looking for a licensed nurse with the authority to practice within Swedish healthcare. Proficiency in Swedish at least at level C1 according to the Council of Europe’s scale is a requirement. You have previous experience working as a nurse, and for this position, it is a requirement that you have a B driver's license.

We consider it meritorious if you have previously worked in any of the following areas:

• Internal medicine
• Infectious diseases
• Inpatient care
• Municipal health and medical care.

• are accustomed to working digitally
• can explain technical solutions in a simple and pedagogical way

As a person, you are confident in your professional role and have the ability to make independent decisions. You are someone who sees opportunities instead of obstacles.
