We are looking for summer substitutes

We are looking for summer substitutes for our case management group within the Social and Care Administration, primarily investigating the right to services for the elderly according to the Social Services Act. As a substitute in the case management group, you will be part of a team of case managers where you will work closely together and have daily contact with colleagues.

JOB RESPONSIBILITIES

As a case manager in elderly care, you will investigate the right to support and services for the elderly so they can live as independently as possible. The work is characterized by a professional approach, legally secure case management, and a holistic perspective on the user's life situation. Main tasks include informing individuals and their relatives about rights, services, and support. You will provide guidance on how to apply for support and describe the process for investigation, decision-making, and follow-up. You will also ensure that the individual understands their opportunities for participation in the case.

In case management, you will receive and register applications and conduct needs assessments based on incoming documentation and information. In the authority group, we use IBIC (Individual Needs in Focus) as an investigation method. To gather information for investigations, individual conversations, home visits, and collaboration with other actors are conducted as needed.

In the exercise of authority, you will assess what services are needed based on the investigation. You will independently make decisions about services in accordance with applicable guidelines, legislation, and case law. You will motivate and document decisions clearly to ensure legal security and transparency. Documentation is done in the operational system Treserva.

Collaboration is an important part of the exercise of authority as it needs to take place with several different actors, such as providers of various municipal services and health care.

QUALIFICATIONS

We are looking for someone with a degree in social work or from a social and care program. You may also be eligible if you have an ongoing social work education with completed field training, preferably within authority exercise. Since you will need to drive a car for work, we require a B driver's license for manual transmission. You should have good knowledge and experience in authority exercise and the documentation requirements in the field. You have good communication skills in the Swedish language, both spoken and written, and possess good computer skills. It is an advantage if you have knowledge of the IBIC method, individual needs in focus, as well as experience in documenting according to the method.
