JOB RESPONSIBILITIES

We are looking for someone who wants to work closely with people and nature while making a real difference every day for our patients. We are now seeking a committed and reliable nurse who wants to become an important part of our rural healthcare.

With us, you will work with home healthcare and meet patients where they feel safest, in their own homes. Many of our patients are unable to get to the hospital or health center on their own and therefore need qualified care at home. Your contribution will be crucial for them to receive the care and support they need.

As a nurse with us, you have a central and independent role with varied tasks. You will meet patients of different ages and life situations, work with great responsibility, and have good opportunities to influence both working methods and the development of the organization.

The work involves both medical interventions and close support to patients and their relatives. You will collaborate with doctors at the health center where the patient is registered, as well as with home care providers to create a safe and cohesive care. Our common goal is to provide the patient with the conditions to stay at home in a safe, secure, and dignified manner.

Your tasks include:

• Performing health and medical care interventions in the patient's home
• Being responsible for care and making medical assessments
• Working in a team with physiotherapists, occupational therapists, and doctors
• Planning and following up home visits
• Guiding and delegating to care staff and students
• Documenting in Treserva and other operational systems
• Working preventively and health-promoting
• Participating in consulting activities during evenings and weekends

The position is mainly daytime work, but the schedule includes evening and weekend shifts. You will then work in our consulting activities, which include the municipality's care and nursing homes, home healthcare, and group homes within LSS and social psychiatry.

QUALIFICATIONS

You are a reliable and structured nurse with a genuine commitment to patient-centered work. In this role, you have a central and significant function for our enrolled patients within home healthcare. You work independently with great responsibility while closely collaborating with other professionals in a team to create safe and cohesive care.

• Licensed nurse
• At least two years of experience as a nurse
• Have a B driver's license and driving experience
• Have documentation experience
• Good knowledge of the Swedish language in speech, writing, and reading comprehension

It is advantageous if you have experience in municipal health and medical care. Knowledge of the operational system Treserva, which is our documentation system, is also a plus.

To succeed in this role, you are:

• Confident in your role as a nurse
• Flexible and can quickly adapt and make decisions based on the patient and their needs
• Proactive and take responsibility for ensuring your tasks are completed
• Structured, good at planning
• Able to prioritize and efficient in using resources to achieve results
• Have good communication skills and adapt your language according to the recipient

Documentation is done in Treserva, planning is done in TES. Lifecare is used for communication between care providers.
