Your Mission
Your area of work with us is psychosocial ill health that does not require the resources of specialist psychiatry. In your role as a social worker, you provide support and advice to strengthen the patient's resources to manage their life situation, such as crisis and grief processing. You work with individual psychosocial mapping and support the patient in navigating among the various support resources in society. You will be offered regular supervision.
You primarily work independently, but also in teams, both with colleagues in the mental health team and with other professional groups at the health center. Your consultations with colleagues at the health center and within primary care are an important part of the job. The age group you encounter at the health center consists of adults from 18 years old.
Your Competence
We are looking for someone who is a licensed health and medical care social worker, with good experience in psychosocial work. Basic training in psychotherapy with a focus on CBT step 1 is an advantage, as well as experience from work in primary care.
To be successful in the role, you need to be self-directed, have good structure in your work, possess a strong initiative, and also have high flexibility. You enjoy developing operations and forms of collaboration, both within and between health centers. Respectful treatment is a high priority in our mission, and we place great importance on personal suitability for the position.
Our Operations
Heby Health Center is part of the management of Close Care and Health, Region Uppsala, and is located in western Uppland with approximately 7,000 registered patients, and we have 30 employees working in teams. Our operations are broad, and all professional areas within the health center's mission are represented. We also have a youth clinic.
Here at Heby Health Center, we strive to have a good work environment with "high ceilings" and satisfied patients. We work in close collaboration between all professions, and a guiding principle is "freedom under responsibility," where there is great opportunity to influence your work situation as long as the goals of the operations are met. The wide variety of patients creates a stimulating and developing workplace. We have diabetes, asthma/COPD, and heart failure nurses with their own clinics and a geriatric clinic.
